Purpose. This form is completed by every employee, panel member and assessor to declare actual, potential or perceived conflicts of interest and to record the manager's decision on how the conflict will be managed.

1.When to complete this form

Complete this form on commencement, annually, before joining any assessment or evaluation panel, before inspecting a provider, and as soon as a new conflict arises. A nil declaration is still a declaration and must be completed.

2.What is a conflict

A conflict exists when you, a family member, a close friend or an associate could gain or lose from a decision you are involved in, or when a reasonable person would think your judgement could be affected. Membership of a community organisation that applies for funding, previous employment with a provider and a relative on a provider's board are common examples.

3.How to complete it

Describe the interest and the decision or process it relates to in plain words. Say whether it is actual, potential or perceived. Suggest how it can be managed, for example by not taking part in the assessment of one applicant. Sign and date the form and give it to your manager before the process starts.

4.Manager's decision

The manager must decide on the management plan, which can include removing you from the process, restricting your access to particular files, or recording the interest and proceeding with supervision. The manager records the decision, signs the form and files it on the process file and the branch register.

5.Records

Declarations are kept on the relevant grant, procurement or compliance file and in the branch conflict of interest register. They are available to auditors and integrity bodies and are retained for at least seven years.
