Purpose. This template is used by each assessor to record the eligibility check, the score and written reasons against every criterion for a grant application, so that the moderation panel and the delegate can rely on a consistent record.

1.Before you begin

Confirm that your conflict of interest declaration for this round is on file and that you have no conflict with this applicant. Read the program guidelines and the criteria weightings for the round. Assess only the information in the application and any clarification recorded on the file.

2.Eligibility

Record the result of each eligibility check with the evidence you relied on. An application that fails any eligibility rule must not be scored; record the reason and refer it to the program manager.

3.Scoring

Score each criterion on the scale in the guidelines, usually zero to ten, and write reasons that explain the score by reference to what the application says. Reasons must be specific enough for another assessor, the delegate or a reviewer to understand the judgement. Do not adjust scores to fit a budget.

  • Outcomes and measurement

4.Overall comment and conditions

Summarise the strengths and weaknesses, flag any risk to delivery, and recommend any conditions such as a revised budget, a milestone report or governance support. State whether the application is recommended, recommended with conditions or not recommended.

5.Record

Save the completed report to the application file, enter the total score in the grants register and forward the report to the moderation panel by the round deadline.
